AGREEMENT
PRE-APPOINTMENT ACKNOWLEDGMENT
Client's hair is at least to their shoulders, and Client's shortest layer is to their cheekbone (minimum) unless previously approved during phone consultation. Current hair photos were sent to stylist, as well as any inspiration photos in advance of appointment date.
*Total price can change if hair is different in person than what was discussed during the consultation, or if the client's wants/needs change during the appointment.
Client understands that Golden Strands Hair Extensions is the hair used for the extension installation method by the stylists at Mykala and Co Salon. Stylist guarantees the hair is always purchased directly from Golden Strands Hair Extensions directly therefore guaranteeing the authenticity of the hair.
If you are entitled to replacement of hair due to a quality issue, it must be replaced within 2 weeks of initial install. Guests traveling to Mykala and Co Salon requiring an "adjustment" within the first 2 weeks, are responsible for their own travel expenses. We do not offer refunds and/or exchanges on services.
APPOINTMENT ACKNOWLEDGMENT
All photos/video taken by Mykala and Co Salon are property of Mykala and Co Salon and can be used for marketing at the discretion of Mykala and Co Salon and its employees.
Client acknowledgment that "Extension at-Home Care Guidelines" was discussed in detail with stylist and that a copy was retained for Client's use.
*Please note, "Extension at-Home care Guidelines" is vital to insure optimal results.
WAIVER AND INDEMNIFICATION
Per my phone or in salon consultation and as memorialized and understood through the PRE-APPOINTMENT ACKNOWLEDGMENT, APPOINTMENT ACKNOWLEDGMENT, and the EXTENSION AT- HOME CARE GUIDELINES, wish to proceed with Natural Beaded Rows hair extensions.
I certify that the information that I supplied to my stylist during the Phone Consultation, PRE-APPOINTMENT ACKNOWLEDGMENT, APPOINTMENT ACKNOWLEDGMENT, and the EXTENSION AT-HOME CARE GUIDELINES are correct. I agree to indemnify and hold harmless Mykala and Co Salon and/or it's employees for loss, damage, claims or actions of any type that I may suffer as a result of failure to provide them with accurate and complete information.
I agree that I will not hold Mykala and Co Salon and/or it's employees liable in any way for any loss, damage, or injury suffered by me as a consequence of my use/or misuse of Natural Beaded Rows hair extensions.
I will indemnify and hold harmless Mykala and Co Salon and/or it's employees free from all loss, damage, claims, or actions of every kind that they incur or that are claimed or brought against them by any party as a consequence of my use of Natural Beaded Rows hair extensions
Payment Policy:
I understand my service is at no set price and that I will be charged a customized amount based on technique and product used. I understand that acceptable methods of payment include cash or credit/debit card. I understand that all services are non-refundable.
SALON GUEST POLICIES:
Children are not allowed at the salon. If you bring a child with you to your salon service, you will kindly be asked to reschedule to another day or time and will be subject to the same-day cancelation fee.
A Deposit will be collected from New Clients in order to reserve a desired appointment time. Color Appointments will require a $50 deposit and New Extension appointment will require a $550 deposit taken by phone. If an appointment is created using our online booking system a deposit of up to $1 will be charged. In the event of a no-show or last-minute cancelation, the fee will be charged to the card used to make the deposit and/or no-show fee.
Appointments that result in a No-Show will be charged 50% of the scheduled service and will be required to secure all future bookings with a deposit.
Cancellations less than 48 hours in advance will result in a $50 fee.
Same-Day Cancellations will result in a $100 fee.
Please do not be more than 15 minutes late to your appointment. If you are later than 15 minutes, you will need to be rescheduled and will be subject to the Same [Day Cancellation fee].
