Due to the 2019-2020 outbreak of the novel Coronavirus (COVID-19), Mykala and Co Salon is taking extra precautions with the care of every client to include health history review and enhanced sanitation/ disinfection procedures in accordance with the Idaho Bureau of occupational licenses.

Symptoms of COVID-19 include:

  • Fever

  • Fatigue

  • Dry Cough

  • Difficulty Breathing

I agree to the following:

  • I understand the above symptoms and affirm that I, as well as all household members, do not currently have, nor have experienced the symptoms listed above WITHIN THE LAST 14 DAYS.

  • I affirm that I, as well as all household members, have not been diagnosed with COVID-19 WITHIN THE PAST 30 DAYS.

  • I affirm that I, as well as all household members, have not knowingly been exposed to anyone diagnosed with COVID-19 WITHIN THE PAST 30 DAYS.

  • I affirm that I, as well as all household members, have not traveled outside of the country, or to any city considered to be a "hot spot" for COVID-19 infections WITHIN THE LAST 30 DAYS.

  • I understand that Mykala Bowlby and Mykala Plus Co LLC cannot be held liable for any exposure to the COVID-19 virus caused by misinformation on this form or the health history provided by each client.

Mykala and Co Salon is following these enhanced procedures to prevent the spread of COVID-19:

  • Additional time included between appointments to prevent client contact with each other.

  • Salon doors will be locked, send your stylist a text when you arrive and we will call you to let you into the building.

  • We will be taking touchless temperatures at the door, anything over 99.8 degrees will be rescheduled.

  • Each client required to wash hands upon arrival

  • Your stylist will thoroughly clean hands and wear gloves during necessary treatments

  • Your stylist will wear a clean mask

  • All skincare product bottles used during treatment will be placed on the trolley for disinfection and disposable items will be used when possible

  • All surfaces will be wiped thoroughly with disinfectant

By Signing below, I agree to each statement above and release Mykala Bowlby and Mykala Plus Co LLC from any and all liability for the unintentional exposure or harm due to COVID-19.

Mykala Bowlby agrees to abide by these standards and affirms the same.

All payments reserved for services will go directly to Mykala and Co. Salon. 

 

It is our number one priority to make you happy with your service and your experience. Any adjustments you wish to have done with your service will need to be resolved within a week of your original service.

No refunds are given as you cannot return services, unless a technical error occured, and that decision is solely decided by the owner (MyKala). If at any point you wish to have a different stylist adjust your service, you will be charged in full by that separate stylist for receiving additional services from them.

We guarantee all work, meaning we are  happy to make any required adjustments if needed.

 

We do not guarantee that any corrective color can be achieved in a single sitting. Corrective color can be anything from having box color, removing old color (professional or at home), chemically damaged hair, chlorine, mineral build up, etc. Corrective color can take anywhere from 2-8 sessions depending on the integrity of the hair.

 

We do not guarantee the longevity of any color service without the purchase of Unite professional products.

 

We understand that your hair is a great investment and we are here if you have any questions.

 

We require 48-72 hour cancellation window BEFORE appointment to avoid the $75.00 (or 50% of service) cancellation fee. We reserve the right to charge 100% of the service amount if appointment is “no-showed”.

 

Any camera footage or photos of your service can be used by Mykala and Co marketing via social media/ youtube.
